We evidently ran into a problem with internet providers blocking the ballot, possibly because
of the attachment and the volume of emails sent. Therefore I have included it in the body of
this followup email. Please fill it out and return it to the address show below.

Dear Jazz Journalists Association member,
It is time for active, professional JJA members to get to work nominating individuals for this year’s Jazz Awards. Please note that corporate and support members of the JJA are not valid nominators or votes in the Jazz Awards, but they are very welcome to participate in other aspects of the Jazz Awards initiative. 
 
The nomination categories are unchanged from last year, with the usual 
stipulation that previous winners of the lifetime awards/one-time 
journalism, broadcast or photography awards are ineligible.
 
You may list up to four nominees per category (if more than four are 
listed, the last one will be discarded).
 
Please email your list of nominations to me at   HYPERLINK

no later than February 28, 2005. Any nominations received after that
